Abstract

|
background :The Global Deterioration Scale (GDS) used for staging of dementia is easy to assess, as it lists specific characteristics on each stage. However, the presence or absence of a single symptom in a patient does not uniquely dictate the GDS stage. So, a computerized, systematic combination is needed for untrained doctors. This study is aimed to develop a simple algorithm for GDS staging.
Methods:We sorted out all the items of examples on GDS and divided them into items appropriate for caregivers (20 items) and for patients (16 items). Subjects older than 50 years who, for memory impairments, visited at one of three neurology clinics and two psychiatric clinics in 4 university affiliated hospitals were included. A structured questionnaire with 20 questions was administered to the subject-informant dyads. After that, psychometricians or well-trained nurses assessed the subject with the 16 items. They also graded the 182 subjects with GDS score as are their usual practice. Classification tree analysis was accomplished by using SPSS AnswerTree 3.0 software.
Results:182 subject-informant dyads were included for analysis. Among the 36 predictors entered, ¡®get lost when travelling¡¯ was the most important predictor of GDS score(¥ö2=96.6, p=0.0000). Starting with ¡®get lost when travelling¡¯, a classification tree algorithm with 13 predicting variables was developed.
Conclusion:A classification tree algorithm for GDS staging was developed and it can be used to narrow the choices range of GDS staging for persons on suspicion of cognitive impairment. Validity and reliability test of this algorithm are needed."
